Eg that 0/4 courtesan with threat at end of conflict vs fierce.
1/1 agent - she loses no health right? 4/4 agent - same 1/1 with +3/+3 fierce played on him - ? 3/1 agent with fierce - ?
Any other scenarios to be aware of?

The 0/4 courtesan does not have threat. She simply kills everyone when blocking and only after combat has resolved. Even when she has died in the process.
Threat only works when being blocked. When an agent with threat gets blocked the opposing agent will die before combat resolution, so the agent with threat is not harmed in any way.
When an agent with threat is blocked by someone who is safe. Threat removes safe and then damage is dealt with normally. So a safe agent can block and kill an agent with threat.
Fierce works when attacking and blocking. It is simply a carry over effect. I.e. any strength higher than the others health will be scored in the region.
Threat and fierce will first kill the blocking agent and then score the full power of the attacking agent...

I seem to recall though that the theat agent dies anyway when blocked by courtesan, almost like an exception to the rule? Or am I getting it confused with Leo who gets killed by her?

She'll kill both. I know she has wiped out my Leo and Lanz before. That's correct and no exception to the rules. Fiora will kill everybody after combat has resolved. Even when she gets killed in the process as the result of the other agent having the threat ability.

She's the exception to my rule (of thumb) playing Leo. Leo never, ever dies against an agent with less attack than his health. Oh wait, there's that courtesan chick....
